I just had to call to say thanks a lot. Carol, the Occupational Therapist, came today for the first time. She was amazing! It went so well. I couldn’t believe it. By the end of the session, Stephanie went to her and played with her. She is usually so shy. The Occupational Therapy session was better than expected”

How Is My Child Developing?

Developmental Checklist

This checklist is a guide of what you can expect of your child from birth to age three. It is based on the work of Gesell, Arnold, and Frances. Be aware that children develop at different rates. Parents who have concerns may wish to discuss them with their doctor, and call their local Early Intervention Program.

Please print out this form, check all boxes that apply, provide us with necessary contact information and any additional comments you may have, and send the completed checklist to our office for further discussion. You can either fax this completed form to us at 845-369-1337 or mail it to us at 23 Olympia Lane, Monsey, New York 10952.
